The SGSA has produced a standard ‘Injuries and medical treatment report form’ template for use by sports grounds to record any injuries and/or medical treatments by ground medical teams. The categories and information used in the form are in line with the required information from football grounds as part of the SGSA’s spectator injury statistics collection.
